All the forms you will need to make this transition as easy as possible are provided here. Simply print and mail

Eligibility Requirements
In order to become a member of the University Credit Union, at the time of application you must be one of the following:
- Persons who Live, Work, Attend School, Worship or Volunteer in Salt Lake County (see map)
- Immediate Family member* of an existing or eligible member
- Employee of the University Credit Union
- Spouses of persons who died while within the field of
membership of the University Credit Union
* Immediate family member is defined as spouse, child, sibling, parent, grandparent, or grandchild. This includes stepparents, stepchildren, stepsiblings and adoptive relationships.
